Executive Summary assignment

For Engineering we were to investigate and write an Executive Summary on the tragic loss of the Space Shuttle Columbia and the seven lives of the astronauts aboard. There are many reasons why the accident occurred, many of which people still do not know.
**
On the morning of February 1, 2003, the Space Shuttle Columbia disintegrated over Texas just before it’s scheduled 28th mission, “STS-107”. This scheduled mission resulted in the loss of seven lives, the crew aboard the spacecraft.

Major unfixed issues were
-Communications with the N.A.S.A team were not functioning correctively and accordingly
-The camera was not in the right spot, looking at the right direction, causing it to be harder to determine the real damage issue of the left wing.
-Shedding of the foam
-Other minor issues

Although there were many unfixed issues about the spacecraft, there were also many fixed ones as well.

Major looked upon issues were
-The temperature, which was increased from 74-degrees Fahrenheit to 80-degrees Fahrenheit
-The water separators would not allow water to flow
-The settings of the 7A Tank Heater would only work automatically and not manually
